This following contact form was created and published in under 5 minutes using the free beta Fast Contact Form creation tool from http://www.fastcontact.com.  The same people who developed FreeFind - the search engine tool for your web site.  I like them both. And the best part about this form, apart from it's ease of use, is that it resists spam bots.

Tutorial:

1. Go to their website and open a unique user account.
2. If you wish to use the form in your own page as I did here, copy the Fast Form HTML code between these two points:

<!--End of FastContact Form -->

3. Then make a text box on your page and using Ctrl+T, paste the code. That's all there is to it. It doesn't get much easier than that. This is a working contact form. Feel free to test it
